New patients

Please note that contacting me through the portal or leaving a phone message, such as to request an evaluation, does not establish a doctor-patient relationship.

Existing patients

Sign in to my SimplePractice patient portal to schedule, modify, or cancel appointments, or to contact me about other concerns; or call 206-717-4800.

Cancellations

Please visit the patient portal or call to cancel your appointment at least 48 hours in advance. Late cancellations or no-shows are billed according to my practice policies.

Directions/Parking

My office is located at 200 1st Ave West, Suite 400, in the Lower Queen Anne neighborhood of Seattle-- close to South Lake Union, Denny Triangle, Belltown, and Westlake. The office is easily accessible by many bus routes. There is street parking available nearby. If you drive to your appointment make sure to arrive at least 15 minutes early to find parking, and check signs for parking fees and restrictions.
